Volunteer Application

Thank you for your interest in volunteering for the 2008 U.S. Mid-Amateur Championship to be held at Milwaukee Country Club and Brown Deer Park, September 6 - 11, 2008. As part of the volunteer package, volunteers receive a uniform (shirt, wind shirt, cap) and a parking pass. A complementary round of golf at Milwaukee Country Club on Monday, September 29 will be given to any volunteer working more than 3 shifts. Volunteers are required to attend one orientation meeting at Milwaukee Country Club on either Wednesday, August 27th or Thursday August 28th with timing to be determined. Thank you in advance for your commitment to volunteering. One application is required per person.
